What is Five Wishes®?
It is a document that allows you to put in writing how you want to be cared for if you are seriously ill and unable to speak for yourself. If you don’t make these decisions now, someone you love may be put in the difficult position of making these decisions for you.

Five Wishes is written in simple English (and Spanish), rather than hard-to-understand doctor and lawyer language. The inclusion of personal and spiritual wishes, and blank lines available to personalize these wishes, allow you to specify such things as whom you would like by your bedside, and whether you would like to have music played, or scripture or beloved poems read to you.

Five Wishes® is legally valid
Thanks to the help of the American Bar Association, Five Wishes is legally valid in 34 states and DC.

Even in those states where it may not be legally valid, it is still used as a helpful tool for expressing how individuals want to be cared for. More than 1,000 organizations in America are distributing Five Wishes.
